Case Management Disclosure

DISCLOSURE OF INFORMATION

Case Management Relationship

  • A third party has requested that we provide case management services on your behalf, and have agreed to pay for the requested case management services
  • You have been assigned a Case Manager to assist you throughout the rehabilitation process, and should refer to our website, www.1strehab.com, ‘Resources’ tab, to learn more about the role of a Case Manager. If you are unable to access our website, your Case Manager can provide the information via paper documents upon request
  • We do not compensate Case Managers based on utilization of services or vendors used, and your Case Manager will remain an objective party throughout the rehabilitation process
  • Although a third party has hired us to provide services, you are your consumer. Your Case Manager must provide services that at all times respect your needs and ensure that your needs are addressed, within the limits of what the third-party payer has authorized

Disclosure of Information to Third Parties/Confidentiality

  • If you are represented, and in the event that your Attorney does not allow your Case Manager to have direct communication with you, such will be coordinated through your Attorney’s office
  • Otherwise, throughout the course of your rehabilitation services, you will likely maintain regular telephonic and/or in-person contact with your Case Manager. Summaries of said contacts/meetings will be forwarded to the appropriate case-involved parties
  • These summaries may include updates regarding rehabilitation progress, significant developments that occurred during the interaction or since the previous update, observations made by the Case Manager, issues encountered during the rehabilitation process, or any other information that currently impacts or may begin to impact the rehabilitation process
  • Your Case Manager will do their best to refrain from sharing personal information that does not impact your rehabilitation
  • During the course of service provision, all records will be kept private except as follows:
    • If a third party (i.e. insurance carrier, attorney, etc.) is paying for services, records will be provided to that party
    • If your Case Manager believes you are going to harm or endanger yourself or others, they are required to notify the endangered individual(s), the proper authorities and/or officials
    • If your Case Manager believes you are going to harm or endanger or abuse children or the elderly, they must report this to state or local authorities
    • If your Case Manager or this agency is sued or court ordered and a properly issued subpoena is received, then information in your file may be released
    • If you are a minor or not your own legal guardian, then the information in your file may be available to your legal guardian or advocate

Notification of Case Management Actions and Recommendations

  • As you, the Consumer, are continuously involved in your ongoing case management activities, you may request information about your Consumer-centered Case Management plan at any time and will be verbally provided with such by your Case Manager
  • Written or electronic notification of case management actions/recommendations may occur if this is a commonly used method of contact between yourself and your Case Manager. However, if the actions/recommendations are discussed verbally, written or electronic notification is not necessary or required
  • A Case Manager may choose to notify you of case management actions/recommendations in writing if it is their preference to do so, or if specifically asked to do so by the third-party payer or a Supervisor
  • Upon case closure, you and your Attorney, if you are represented, will receive notification of such within two days of the Case Manager receiving a case closure request from the third-party payer

Rights and Responsibilities

We adhere to standards that promote the autonomy of Clients and support Client and Family/Caregiver education and informed decision-making. You, as the Client, have the right to be involved in all choices regarding the services you receive.

All of our employees comply with applicable Federal civil rights laws and do not discriminate against, exclude, or treat people differently on the basis of race, color, ethnicity, national origin, age, language, physical or mental disability, religion, sex, sexual orientation, and gender identity or expression.

You have the right to:

  • Receive education regarding your rights and responsibilities;
  • Be involved in all choices that are made regarding the services you receive;
  • Establish alternative approaches to communication when you or a Family/Caregiver are unable to fully participate in the assessment phase;
  • Communicate freely with your Case Manager and Treatment Provider(s) and have input regarding your current health status, treatment options, recommendations and rehabilitation plan;
  • Refuse treatment or services, including case management, and recognize that there may be possible implications of such refusal relating to benefit eligibility and/or your health outcomes;
  • Use end of life and advance care directives, as applicable;
  • Be informed when and how your Case Manager’s services may be changed or discontinued, based on governing law and/or insurance carrier instructions; upon closure of your case by us, you will receive verbal or written notification of the rationale and any instructions for your continued well-being; and/or
  • File a complaint, directed to our Quality Management Program Coordinator via telephone (888) 252-0368, via email: skieley@1strehab.com, or in writing to:

Plain Language Policy

This is viewed as an obligation in promoting clear understanding and avoiding misunderstanding, as well as a facilitator of good results for all parties. These Client materials include written documents provided, as well as information contained here in this section of the website.

We are committed to working in-step with suggestions made by the ‘‘Plain Writing Act of 2010,’’ and strives to ensure all communication from us be SMART: Specific, Measurable, Achievable, Realistic, and have a Timeframe.

Technology Safety Resources/Tips

Passwords

  • Safest passwords contain a combination of letters, numbers, and symbols; avoid using words in the dictionary, important dates, or names of close family members
  • Avoid using the same password for every account you log into
  • Change your passwords frequently

Social Networks

  • Always check privacy settings for your accounts and ensure the account is set to the level of privacy you want
  • Do not “befriend” strangers or others’ whose accounts seem incomplete/unreal on social media

Online Accounts – Practice caution when opening a new online account with any entity, and make sure to fully read their privacy policy

  • Opt out of sharing personal information that is not required to open the account
  • Avoid using the same username and password for all online accounts
  • Ensure that you are logged off when the online account is not being utilized

Safe Web Browsing

  • Ensure that your computer is running current anti-virus and anti-spyware programs
  • Periodically run scans on your computer, separate from your regular antivirus/anti-spyware
  • Occasionally delete your history, cookies, temporary internet files, and saved forms/passwords from your web browser
  • Always be aware of your surroundings when utilizing a web browser; others may be able to view the content of your computer when utilizing a public connection, or may be able to view your screen when sitting in a public area
  • Always log off accounts and devices when not utilizing them

Mobile Telephones/Devices

  • When speaking on your phone in public, avoid topics that are of a confidential nature and be aware that others around you may be able to hear your discussion
  • Keep your mobile device locked at all times it is not in use, and protect with fingerprint technology or a strong pass code
  • Take great care when sharing your location with others from your mobile device’s applications
  • Pay close attention to security and privacy settings for each application on your mobile device
